Discharge planning is the process used to decide what a patient needs for a smooth transition from one level of care to another. Only a doctor can authorize a patient’s release from the hospital, but the actual process of discharge planning can be completed by our Discharge Planner. Ideally, and especially for the most complicated medical conditions, discharge planning is done through a team approach.
Our discharge planning team is available to assist you with the following:
• Admittance to Swing Bed
• Assisted Living, Group Home or Nursing Home Placement
• Home Health Care and Hospice
• Advance Directives (Living Will or Durable Power of Attorney for Healthcare)
• Meals on Wheels
• Medical Equipment (i.e. Walkers, Wheelchairs, Hospital Beds)
