It’s Time for Back-to-School Wellness Exams & Sports Physicals

At Cumberland Healthcare we offer a full spectrum of back-to-school services so your child can be their best, every step of the way. You may schedule an appointment for your child with family practice physicians or advanced practice providers (NP/PA).  A well-child visit may include a weight and height check along with body mass index (BMI), vision, hearing, blood pressure, and developmental milestones. We also have direct access to the Wisconsin Immunization Registry and will update your child’s immunizations accordingly.

If your child participates in school sports, a wellness exam or back-to-school exam qualifies as sports clearance.  If you need a WIAA athletic form, click here for a WIAA form.

Please bring your completed athletic form signed by a parent/guardian. Parents/guardians are strongly encouraged to accompany athletes to their appointments.
