The Health Information Department maintains all patient healthcare records and reviews all requests for release of these records before processing.  The information can only be released with authorization from the patient or their legal guardian.

If you wish to receive a copy of your health records or have your records transferred to/from another facility, please download and fill out the document below.  This document will then need to be brought to or faxed to Cumberland Healthcare.  The fax number to Cumberland Healthcare Health Information department is 715-822-7151.  Please bring a photo ID if you choose to pick up the medical records in person.
